Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Texas.

📋 Description

• Oversee patient census utilizing a solution-oriented approach to address gaps in both clinical and non-clinical patient care.

• Identify and facilitate access to community resources that can enhance patient care.

• Deliver patient education and promote health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, performing reconciliation, ensuring adherence, and coordinating refills.

• Aid in the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster an open dialogue to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• A valid Compact/Multi-State license (LPN) (LVN) is required.

• Proficient in Microsoft Office Suite.

• Exceptional written and verbal communication abilities.

• Experience with Epic Systems (preferred).

• Bilingual skills (preferred).

• Additional Single State licenses (preferred).
