Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Oversee patient census utilizing a resolution-focused strategy to address gaps in both clinical and non-clinical patient care.

• Identify and connect community resources that can enhance patient care.

• Deliver patient education and promote health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, reconciliation, adherence, and coordinating refills.

• Assist in ensuring prompt delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster an open dialogue to better understand their requirements.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency in Microsoft Office Suite

• Outstanding written and verbal communication abilities

• Experience with Epic

• Bilingual

• Additional Single State licensures
