Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Montana.

📋 Description

• Oversee patient census with a focus on resolving issues in both clinical and non-clinical patient care.

• Identify and facilitate access to community resources that would enhance patient care.

• Deliver education and promote health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes identifying potential medication issues, conducting reconciliations, ensuring adherence, and coordinating refills.

• Help ensure the prompt delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster open communication to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Proficient in Microsoft Office Suite

• Strong written and verbal communication abilities

• Experience with Epic

• Bilingual capabilities

• Additional licensures for Single State
