This is a fully remote position, open to applicants in Missouri.

📋 Description

• CareHarmony’s Care Coordinators (LPN) (NLC) (LVN) collaborate extensively with healthcare providers to implement value-based care management initiatives for patients.

• CareHarmony is in search of a skilled Licensed Practical Nurse – LPN Nurse (LPN) (NLC) (LVN) with no less than 3 years of hands-on patient-facing experience, who excels in a dynamic environment, is self-driven, possesses exceptional attention to detail, and recognizes the significance of their role in enhancing a patient’s healthcare experience.

• The ideal candidate will be adept at identifying resources and coordinating care for patients managing chronic conditions.

⛳️ Requirements

• Oversee patient census with a focus on resolving issues to close gaps in both clinical and non-clinical patient care.

• Identify and coordinate community resources that would enhance patient care.

• Deliver patient education and promote health literacy regarding the management of chronic illnesses.

• Manage medication, including recognizing potential medication issues, performing reconciliations, ensuring adherence, and coordinating refills.

• Help ensure the timely provision of services to patients, including Home Health, DME, Home Infusion, and other essential services.

• Address patient inquiries and foster open communication to better understand their needs.

• Assist with referrals and appointment scheduling.

• Active Compact/Multi-State license (LPN) (LVN).

• Proficiency in Microsoft Office Suite.

• Strong written and verbal communication abilities.

• Experience with Epic.

• Bilingual capabilities.

• Additional Single State licensures are a plus.
