Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Oversee patient census with a focus on solutions to address gaps in both clinical and non-clinical patient care.

• Identify and connect community resources that would enhance patient care.

• Educate patients on health literacy and the management of chronic health conditions.

• Conduct medication management tasks, including recognizing potential medication issues, reconciliation, adherence, and coordinating refills.

• Aid in the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster an open dialogue to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Proficiency in technical skills, particularly with Microsoft Office Suite

• Exceptional written and verbal communication abilities

• Experience with Epic (a plus)

• Bilingual capabilities (a plus)

• Additional Single State licenses (a plus)
