Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical patient care.

• Identify and connect patients with community resources that can improve their care.

• Educate patients and enhance their health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, performing reconciliation, ensuring adherence, and coordinating refills.

• Help guarantee the timely provision of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster an open dialogue to comprehend their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Valid Compact/Multi-State license (LPN) (LVN)

• Proficient in technical skills, particularly the Microsoft Office Suite

• Strong written and verbal communication abilities

• Experience with Epic (considered a plus)

• Bilingual proficiency (considered a plus)

• Additional Single State licensures (considered a plus)
