Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Maine.

📋 Description

• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical patient care.

• Identify and connect patients with community resources that can support their healthcare needs.

• Deliver patient education and promote health literacy regarding the management of chronic illnesses.

• Conduct medication management tasks, which include recognizing potential medication issues, performing reconciliations, ensuring adherence, and coordinating refills.

• Aid in the prompt delivery of services to patients, including Home Health, Durable Medical Equipment (DME), Home Infusion, and other essential services.

• Address patients' inquiries and foster open communication to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Valid Compact/Multi-State license (LPN) (LVN).

• Proficient in technical skills, particularly with Microsoft Office Suite.

• Strong written and verbal communication abilities.

• Experience with Epic software.

• Bilingual proficiency.

• Additional Single State licensures are a plus.
