Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee patient census utilizing a resolution-oriented strategy to address gaps in both clinical and non-clinical patient care.

• Identify and facilitate access to community resources that could enhance patient care.

• Deliver education to patients and promote health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, performing reconciliation, ensuring adherence, and coordinating refills.

• Aid in the prompt delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster open communication to better understand their needs.

• Support and manage referrals and the scheduling of appointments.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Proficient in technical skills, specifically with Microsoft Office Suite

• Strong written and verbal communication abilities

• Experience with Epic (preferred)

• Bilingual skills (preferred)

• Additional Single State licensures (preferred)
