Care Coordinator, LPN, LVN

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical patient care.

• Identify and connect patients with community resources that may improve their care.

• Offer education to patients on health management and literacy concerning chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, performing reconciliations, ensuring adherence, and coordinating refills.

• Aid in the timely provision of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster open communication to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Valid Compact/Multi-State license (LPN) (LVN).

• Proficient in Microsoft Office Suite.

• Exceptional written and verbal communication abilities.

• Experience with Epic (preferred).

• Bilingual skills (preferred).

• Additional Single State licenses (preferred).
