This is a fully remote position, open to applicants in United States.
• Assists in care management operations and collaborates with teams assigned to members to ensure that healthcare providers and partners deliver services, achieving continuity of care and member satisfaction.
• Engages with members through outreach via phone or home visits, documenting the care/service plan and associated activities.
• Conducts outreach to members through phone calls or home visits to involve them in discussing the care/service plan, including next steps, resources, questions or concerns regarding recommended care, and ongoing education as needed.
• Coordinates care activities in alignment with the care/service plan and collaborates with healthcare and community providers, as well as members and caregivers, to adapt to any changes or progress as necessary.
• Acts as a resource for various member and/or provider inquiries, requests, or concerns pertaining to the care/service plan.
• Communicates with care managers, practitioners, and others as necessary to facilitate member services and ensure continuity of care/service.
• May assist in conducting service assessments/screenings for members and documenting their care needs.
• Aids in documenting and maintaining member records in compliance with state and regulatory requirements and distributing them to providers as necessary.
• Adheres to standards of practice and policies that comply with contractual obligations, regulatory guidelines, and standards.
• Capable of identifying needs and making referrals to Care Managers, community-based organizations, and Disease Managers.
• Provides education regarding available benefits and resources.
• Performs additional duties as assigned and complies with all policies and standards.
• A High School diploma or GED is required.
• 1 – 2 years of relevant experience is necessary.
• A Bachelor's degree in a social science or health-related field, or a high school diploma with a minimum of one (1) year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health issues.
• This role is classified as safety-sensitive in Arkansas and mandates a driver's license, child and adult maltreatment checks (prior to hire and periodically thereafter), and a drug screening (upon hire and periodically thereafter).
