This is a fully remote position, open to applicants in New Mexico.
• Facilitates the care of individual clients through assessment, care planning, and evaluation.
• Encourages the effective use of clinical and financial resources to enhance care quality and member satisfaction.
• Offers care coordination for members experiencing behavioral health conditions.
• Performs thorough health risk assessments and comprehensive needs evaluations.
• Communicates and formulates care plans, ensuring that services are delivered appropriately.
• Oversees strategies for members and their families aimed at improving health outcomes.
• Serves as an advocate for members' care requirements and identifies any gaps in care.
• Collaborates with the interdisciplinary care plan team to implement suitable interventions.
• 3-5 years of experience in Social Work, Nursing, or a healthcare-related field, or relevant experience in place of a degree.
• Required experience in utilization management, quality assurance, home or facility care, community health, long-term care, or occupational health.
• Proficient in analyzing trends using decision support systems.
• Business management skills, including cost/benefit analysis, negotiation, and cost containment.
• Knowledgeable in referral coordination to community and private/public resources.
• Requires in-depth understanding of cost-effective care coordination.
• Capable of making decisions that necessitate substantial analysis and investigation.
• Ability to determine suitable courses of action in complex scenarios.
• Competent in maintaining complete and accurate enrollee records.
• Strong verbal and written communication skills.
• Ability to collaborate effectively with clinicians, hospital officials, and service agency contacts.
