This is a fully remote position, open to applicants in Illinois.
β’ Conducts visits to Medicaid members in their residences, Assisted Living Facilities (ALFs), Long-Term Care Facilities, and other care environments β 75-90% local travel (refer to the Additional Information section).
β’ Ensures that members receive services in the least restrictive environment to achieve and/or maintain optimal well-being by evaluating their care requirements.
β’ Identifies and addresses obstacles that impede effective care.
β’ Develops and implements strategies to satisfy care needs.
β’ Coordinates services, monitors, and assesses the case management plan in relation to the member's personal objectives.
β’ Guarantees that the member progresses towards desired results by consistently monitoring patient care through assessments, data collection, discussions with the member, and proactive care planning.
β’ Directs members and their families to suitable resources for their care.
β’ Bachelor's degree in social sciences, social work, human services, or a related discipline.
β’ A valid, unrestricted Licensed Practical Nurse (LPN) license in the state of Illinois, along with one (1) year of experience in performing comprehensive assessments and providing formal services to elderly individuals.

β’ A minimum of one (1) year of experience in healthcare and/or case management.
β’ A minimum of one (1) year of experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), and/or managed care organizations.
β’ Proficiency in utilizing various electronic information applications/software programs including electronic medical records.
β’ Willingness to travel within the region to meet face-to-face with members and/or their families, community partners, and care teams.
