This is a fully remote position, open to applicants in Florida.
• The Entry-Level Care Coordinator (Intake Coordinator) acts as a vital contact point for patients entering care, assisting the clinical team by gathering essential information needed for the initial bio-psycho-social evaluation.
• This position plays a key role in facilitating the initial intake appointment and collecting pertinent patient history and background information to enhance the clinical assessment process.
• Intake activities are conducted in collaboration with and under the guidance of licensed clinical staff to guarantee proper evaluation and continuity of care.
• The Care Coordinator fosters a welcoming atmosphere for patients while ensuring that intake documentation is thorough, precise, and accessible to providers before the clinical evaluation.
• Conduct structured intake appointments with new patients to gather data relevant to the bio-psycho-social evaluation process.
• Collect and document patient information such as: demographic details, presenting issues, psychosocial background, family and social history, educational and occupational background, previous behavioral health treatment, current medications, and basic health history.
• Accurately document the gathered information within the electronic medical record (EMR) to aid licensed providers in completing the clinical evaluation.
• Collaborate with licensed clinicians to facilitate the intake and evaluation process. Ensure that pertinent patient history and intake data are available for provider review.
• Relay important information obtained during intake to the relevant clinical staff.
• Organize and schedule initial evaluation appointments with the appropriate providers. Ensure that necessary intake forms, questionnaires, and consent documentation are finalized prior to the patient's evaluation appointment.
• Confirm patient demographic and insurance details during the intake process. Upload, organize, and maintain patient documentation within the EMR. Ensure that intake records are completed promptly to support clinical workflow.
• Provide a friendly and supportive experience for patients entering care. Respond to basic inquiries regarding services, scheduling, and intake procedures. Escalate urgent concerns or clinical issues to licensed providers or clinical leadership.
• Adhere strictly to HIPAA and patient confidentiality requirements. Follow organizational policies regarding documentation, intake procedures, and patient communication.
• A Bachelor’s degree is required, preferably in psychology, social work, human services, healthcare administration, public health, or a related discipline.
• This is an entry-level position; prior experience in healthcare, behavioral health, social services, or customer service is preferred.
• Experience with patient intake, scheduling, or care coordination is advantageous.
• Strong interpersonal and communication abilities are essential.
• Capability to establish rapport with patients from various backgrounds.
• Excellent active listening and interviewing skills.
• Ability to collect sensitive information in a professional and empathetic manner.
• High attention to detail and accuracy in documentation is critical.
• Ability to function effectively in a fast-paced clinical environment.
• Basic computer skills and a willingness to learn electronic medical record systems.
