This is a fully remote position, open to applicants in United States.
β’ You will play a key role in influencing and ensuring optimal clinical care for patients by providing outstanding patient service and care coordination across various disciplines.
β’ Utilizing an integrated technology platform, you will be supported by a comprehensive interdisciplinary team that includes Nurse Practitioners, Nurse Care Managers, Dieticians, Pharmacists, Licensed Clinical Social Workers, Population Health Social Workers, and Psychiatrists.
β’ Your responsibilities will include spearheading comprehensive care coordination with an emphasis on identifying and addressing Social Determinants of Health while collaborating with the interdisciplinary care team.
β’ You will serve as a vital resource for patients, assisting them in maneuvering through the healthcare system, accessing necessary resources and support, and achieving improved health outcomes.
β’ A minimum of 3 years of experience in healthcare coordination, community health work, or a related healthcare field.
β’ Proven experience with a team-based approach to care.
β’ Extensive knowledge of community resources.
β’ Exceptional ability to prioritize tasks and switch between them to meet the needs of patients and the overall care team.
β’ Self-motivated and capable of resolving issues in a professional and innovative manner.
β’ Strong communication and teaching abilities.
β’ Intermediate proficiency with MS Office Suite, including Outlook and Teams.
β’ Capability to work effectively in a mainly remote setting: A minimum download speed of 25 Mbps and an upload speed of 10 Mbps is required; a wired connection to the home internet (Cable, Fiber, or DSL) and hardwired connection to the internet device is recommended.
β’ Team Members must operate from a dedicated space free of ongoing interruptions to safeguard member PHI / HIPAA information.
