This is a fully remote position, open to applicants in Tennessee.
• Collaborating with members and their families to pinpoint necessary supports and direct services aimed at achieving personal health, employment, and independent or community living goals.
• Working alongside a team of clinical and social support professionals to address the physical, behavioral health, and long-term service requirements of each member.
• Conducting comprehensive and impartial face-to-face visits to evaluate each member's situation, determining their current status and needs, including physical, behavioral, functional, psycho-social, financial, and expectations for employment and independent living.
• Applying criteria to authorize suitable home and community-based services, ensuring that these services are delivered and that the needs of the members are adequately met.
• A minimum of 3 years of experience in a clinical environment.
• Registered nurse with a valid license in the state of Tennessee, or possess a license in the state of residence if it participates in the Nurse Licensure Compact Act; alternatively, a Master's in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
• Outstanding customer service abilities.
• Must reside in Shelby County, Tennessee.
• Availability for an 8:00 am – 5:00 pm EST schedule (no on-call responsibilities).
• A valid Driver's License is required.
• TB Skin Test (applicable to coordinators who operate in the field).
• Various immunizations and/or associated medical tests may be necessary for this position.
• Digital literacy assessment.
• Opportunity to work a compressed work week after one year.
