This is a fully remote position, open to applicants in Kansas, +1 more state.
• Lead ongoing caregiver support groups (both cohort-based and drop-in), adhering to Trualta's group structure and materials.
• Establish a secure and organized environment for caregivers to exchange thoughts, pose questions, and offer mutual support.
• Record significant themes or issues raised in each session (attendance is monitored separately in Looker).
• Prepare for sessions as necessary—some groups are anonymous while others are not, which may influence your preparation.
• Adhere to Trualta's escalation protocol to highlight urgent caregiver needs or risks (such as burnout crises or safety issues) to the Caregiver Success Manager.
• Keep sessions centered on peer support and education—redirect personal medical or treatment inquiries to the caregiver's own healthcare team instead of providing clinical advice.
• Uphold the confidentiality of all information shared by caregivers in groups, in line with Trualta's confidentiality policy.
• Stick to your designated weekly schedule and notify of any changes with adequate notice.
• Licensed Non-Physician Practitioner (NPP), which includes: clinical nurse specialist (CNS), certified nurse-midwife (CNM), physician assistant (PA), clinical social worker (CSW), marriage and family therapist (MFT), mental health counselor (MHC), physical therapist (PT), occupational therapist (OT), speech-language pathologist (SLP), or psychotherapist.
• Experience in working directly with patients and families facing chronic illness, disability, or aging-related care challenges.
• Comfortable leading group discussions, preferably in a virtual/video format.
• Personal caregiving experience (whether formal or informal) is preferred.
• Proficient in using digital platforms (such as video conferencing and basic documentation tools).
• Reliable availability for the assigned weekly schedule.
