This is a fully remote position, open to applicants in California.
• Assist our Care Delivery initiative (WiderCare+), utilizing the approved curriculum to foster trust and enhance health.
• Implement personalized care strategies for patients facing serious, high-risk health conditions.
• Serve as the intermediary between the patient and the billing professional.
• Perform person-centered evaluations to gain insight into the patient’s social and cultural background.
• Help patients and caregivers navigate the healthcare system in California.
• Support patient communication tools and automation processes.
• Connect patients to supportive services and community resources.
• Employ motivational interviewing and capacity-building techniques to assist patients in managing stress.
• Be prepared to complete and uphold all CMS-mandated training and applicable state requirements, including competencies in interpersonal relationships, service coordination, and professional ethics.
• Possess over 3 years of relevant experience in community outreach, facilitation, or healthcare. Experience in navigating high-risk chronic conditions or complex care management is highly advantageous.
• Demonstrate strong computer proficiency and the ability to seamlessly navigate web-based and app-based platforms.
• Have a comprehensive understanding of the socio-economic and public health challenges faced by marginalized populations in California.
• Exhibit excellent written and verbal communication skills, with a personable demeanor and the capability to motivate and influence diverse individuals.
• Hold a high school diploma/GED (a college degree is preferred). Must possess a valid driver's license, reliable transportation, and a flexible schedule for occasional work outside of standard business hours.
