Cardiometabolic Specialty Care Advanced Practice Provider

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform comprehensive assessments of patients with hypertension through virtual consultations, which includes ordering necessary diagnostics, interpreting lab and imaging results, and creating treatment plans in partnership with the specialty care clinic team.

• Develop precise diagnoses and tailor treatment plans for patients, incorporating medication management, lifestyle changes, and monitoring suggestions.

• Proactively reduce the risk of cardiovascular disease by initiating and managing statin therapy for individuals with diabetes and/or cardiovascular conditions.

• Work alongside an interdisciplinary care team—comprising primary care providers, specialists, and Devoted team members such as pharmacy, social work, and health coaches—as well as family members and caregivers to ensure coordinated, patient-centered care and implement a collaborative care strategy.

• Act as the clinical advisor and furnish clinical escalation support for specialty clinic staff and other teams during business hours.

• Engage in regular panel review meetings to provide insights and guidance concerning medical management.

• Utilize our proprietary electronic health information system for patient visits while also offering feedback to enhance the interface.

• Keep accurate and current patient medical records, ensuring adherence to applicable legal and ethical standards.

• Take part in quality improvement initiatives and ongoing professional development to remain up-to-date on best practices and advancements in hypertension and atherosclerotic cardiovascular disease risk reduction.

• Comply with all relevant laws, regulations, and industry standards, including those related to patient privacy and telehealth.

⛳️ Requirements

• An active and clear RN and APRN or Physician Assistant license must be maintained while employed at Devoted Medical, with a commitment to obtain and maintain licenses in multiple states.

• Over 5 years of experience in outpatient clinical practice, including at least 2 years focused on primary care or a relevant subspecialty (e.g., cardiology, nephrology, endocrinology), with a strong emphasis on hypertension and hyperlipidemia management.

• Solid clinical knowledge in the primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD), encompassing disease processes, evidence-based treatment protocols, medication management, and lifestyle changes.

• Availability to work Monday to Friday from 8:30 AM to 5:00 PM MST or PST is preferred; EST and CST candidates will also be considered.

• Familiarity with AI-assisted clinical workflows.

• Proficient in telehealth technology and electronic health records (EHR).

• Preference for candidates with virtual care experience and a strong commitment to continuing clinical practice through virtual visits, aligning with the mission of delivering care where patients reside.

• Understanding of managed care is advantageous, including the ability to assess STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps.

• Proficiency in both English and Spanish is preferred for this role.
