This is a fully remote position, open to applicants in United States.
• Develop and sustain relationships with case managers, social workers, nurses, discharge planners, and other clinical contacts within hospitals and facilities.
• Gather clinical updates, treatment plans, expected length of stay, discharge planning details, and any barriers to discharge.
• Examine available medical records and compile pertinent clinical information.
• Record clinical updates, communication efforts, discharge details, outstanding items, and subsequent steps in specified systems.
• Liaise with prison medical teams when clarification is needed regarding treatment plans, discharge requirements, or care coordination inquiries.
• Facilitate communication concerning transfers, direct admissions, facility acceptance, and physician-to-physician coordination as necessary.
• Work in partnership with Correctional Account Coordinators, Account Managers, medical records teams, and other internal parties to guarantee timely and accurate case updates.
• Keep case information current in AXIS, Salesforce, SharePoint, spreadsheets, and other designated platforms.
• Finalize documentation and case updates after discharge or transfer is completed.
• Compile and distribute weekly case management reports to the designated contacts at the prison.
• Raise issues related to clinical deterioration, unclear treatment plans, discharge obstacles, communication deficiencies, or urgent matters to management and relevant stakeholders.
• Valid and unrestricted Clinical Social Work license.
• Experience in clinical case management, utilization management, discharge planning, inpatient care, correctional healthcare, managed care, or complex care coordination.
• Solid comprehension of medical terminology, inpatient care, discharge planning, and clinical documentation processes.
• Capability to review, interpret, and summarize medical records along with verbal clinical updates.
• Excellent written and verbal communication abilities.
• Proficiency in managing multiple active cases with conflicting priorities.
• Strong organizational skills, attention to detail, and commitment to follow-through.
• Ability to identify clinical or operational issues that necessitate escalation.
• Familiarity with Microsoft Teams, SharePoint, Salesforce, electronic documentation systems, spreadsheets, and associated technological platforms.
• This role operates on a weekday schedule from Monday to Friday, 9:00 AM to 6:00 PM.
• Dedication to maintaining confidentiality and the proper management of sensitive medical and correctional healthcare information.
