This is a fully remote position, open to applicants in California.
• Collaborate with a dynamic team of healthcare professionals, including health coaches, physicians, the chief medical officer, and other team members.
• Perform health assessments that encompass a review of laboratory results, medical history, and psychosocial background.
• Evaluate symptoms and provide appropriate treatment while working in conjunction with the primary care provider of Twin Health members.
• Demonstrate willingness to provide care via telemedicine and maintain documentation within Twin’s clinical platform.
• Oversee a high-risk population and partner with the care team and members to comprehend social determinants of health and specific population needs.
• Show eagerness to learn and grasp the Twin Model of Care to assist in reversing diabetes and other chronic conditions.
• Possess a foundational understanding of business objectives and service level agreements that contribute to financial, clinical, and quality success and outcomes.
• Work with the Twin medical team to deliver outstanding customer service, centering member care around various chronic conditions.
• Educate patients to encourage habits that prevent diseases and promote good health as outlined by the Twin Health Program.
• Review and discuss patients' medical history, symptoms, allergies, and current medications.
• Inquire about situation-specific questions to evaluate symptoms.
• Prescribe appropriate medications to patients, providing detailed dosage and administration instructions in accordance with Twin Health Policy.
• Keep precise records of patients' contact information, medical history, prescribed medications, allergies, diagnoses, and progress.
• Carry out additional duties as assigned.
• Current Physician Assistant CA state license, preferably with licensure in multiple states.
• Bilingual proficiency in English and Spanish.
• 3-5 years of experience as an advanced practice provider.
• Clinical experience and a strong passion for working with diverse populations, including underserved groups.
• Proven experience in managing patients remotely across various geographic locations and states.
• Possession of 5 or more licenses with readiness to acquire additional ones.
• Enthusiasm for collaborating with a dedicated team of internal medicine and family medicine physicians and health coaches.
• Board certification in family or internal medicine.
• Demonstrated experience working as an advanced practice provider.
• Strong medical knowledge.
• Capability to consult with patients through virtual communication methods.
• Excellent analytical and problem-solving abilities.
