This is a fully remote position, open to applicants in United States.
β’ Assisting members in feeling relaxed and comfortable with video onboarding ahead of their telehealth appointment with a Devoted Medical provider, when necessary.
β’ Collaborating with our home health partners to arrange durable medical equipment (DME) and skilled nursing services.
β’ Facilitating members' access to lab testing and imaging, while meticulously tracking results to ensure review by the ordering clinician when appropriate.
β’ Informing members about lab results and assisting in organizing follow-up care as directed by the clinician.
β’ Coordinating with DM Providers on the collection of medical records, information gathering, appointment scheduling and confirmation, as well as managing communications from Devoted Medical.
β’ Acting as an advocate for our members when they contact Devoted Medical regarding care coordination concerns.
β’ MA Certification (mandatory); expired MA certifications are acceptable!
β’ Fully bilingual (English/Spanish), in addition to English proficiency.
β’ Two (2) years of experience in a patient-facing role, with additional preference for those with telehealth experience.
β’ Previous experience in primary care is advantageous.
