This is a fully remote position, open to applicants in Pennsylvania.
• Facilitates and enhances mental health care for patients participating in Collaborative Care Management (CoCM).
• Delivers evidence-based, short-term interventions, addresses case management requirements, and serves as a vital member of the CoCM team.
• Evaluates and assesses patients for prevalent mental health issues, while monitoring the effectiveness of treatments.
• Shares recommendations with Primary Care Providers.
• Monitors and documents patient follow-up and clinical results in an EPIC-based registry.
• Develops relapse prevention strategies with patients who are in remission.
• Bachelor's Degree in social work, psychology, counseling, nursing, or a related mental health discipline.
• A minimum of 2 years of experience in mental health or behavioral health settings.
• Formal education or specialized training in behavioral health is required.
• Proven knowledge and experience in the behavioral health sector.
• Capability to effectively engage with individuals from diverse backgrounds.
• Must demonstrate a high level of emotional intelligence.
• Familiarity with evidence-based interventions such as Motivational Interviewing, Problem Solving Techniques, and/or Behavioral Activation.
