This is a fully remote position, open to applicants in Tennessee.
• Oversee a diverse caseload of ongoing members alongside short-term cases.
• Deliver thorough care coordination and case management for dual-eligible members facing complex behavioral health challenges.
• Collaborate as part of an integrated care team to align both behavioral and physical health services.
• Work closely with members, caregivers, providers, and community resources.
• Ensure a seamless and coordinated care experience.
• Assist members in navigating their Medicare and Medicaid benefits.
• Advocate for enhanced health outcomes, improved access to care, and an overall better quality of life.
• Provide support for utilization management functions concerning complex and non-routine cases as required.
• Act as a liaison among members, providers, and customers to facilitate healthcare delivery and benefits programs.
• Manage highly complex cases to guarantee the effective execution of interventions.
• Conduct case management activities in community settings, including face-to-face interactions with members as needed.
• A current, active, unrestricted Tennessee license in Nursing (RN) or a behavioral health field at the Master's level or higher is required.
• A minimum of 3 years of experience in clinical behavioral health or substance use disorder is required.
• At least 1 year of experience is essential, with knowledge of community care resources and the various levels of behavioral health care available.
• Proficiency in Microsoft Office applications (Outlook, Word, Excel, and PowerPoint) is necessary.
• Ability to make independent, sound decisions and solve problems effectively.
• Exceptional oral and written communication skills.
• Strong interpersonal and organizational abilities.
• Robust analytical skills.
• Capability to build positive relationships and engage diverse populations in health behavior motivation.
• Experience with Motivational Interviewing Techniques and an understanding of Adult Learning Styles is preferred.
