Behavioral Health Care Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Primarily operate in a virtual environment to assist individuals with both diagnosed and undiagnosed mental health conditions and substance use disorders, along with unmet social service needs.

• Conduct initial and ongoing virtual outreach utilizing an omnichannel strategy to engage a qualified population, aiming to comprehend their needs and barriers to care.

• Employ motivational interviewing techniques to gain insight into the individual's care aspirations.

• Assess for unmet behavioral health, social service, and physical health conditions and requirements.

• Recognize clinical needs and triage escalations, providing brief interventions when necessary.

• Identify and tackle barriers to definitive behavioral health treatment.

• Determine the suitable level of evidence-based care for the individual, as relevant.

• Assist in guiding the individual to appropriate care, including behavioral health and/or primary care, as necessary.

• Facilitate post-care follow-up.

⛳️ Requirements

• A minimum of 2 years of experience in roles such as Community Health Worker, Social Worker, or in a patient-facing customer service healthcare setting; active certification as a Community Health Worker or similar credential, or a licensed social work designation is highly preferred.

• 1-2 years of experience in patient outreach and engagement or a similar background focused on building trustworthy patient relationships.

• Over 2 years of experience in a metric or quota-driven environment and/or with high-volume targets.

• Access to a quiet, HIPAA-compliant, and internet-connected space to perform clinical responsibilities.

• Proven professional or personal lived experience working closely with individuals facing complex chronic needs, homelessness, mental health conditions, or substance use issues.

• Experience in fast-paced, rapidly evolving work settings.

• A candidate who can excel in a dynamic, fast-paced startup environment, demonstrating the ability to navigate uncertainty and embrace challenges with resilience and adaptability.
