This is a fully remote position, open to applicants in United States.
• Accept patient transfers from an Intake Care Manager and continue to provide care.
• Build rapport with patients while creating a secure therapeutic environment.
• Assist patients in gaining insight into themselves, their behaviors, and their relationships.
• Support patients in managing challenges and triggers to enhance relapse prevention and lower risk.
• Foster work skills and encourage participation in social, recreational, or therapeutic activities through brief therapeutic interventions, including motivational interviewing, behavioral activation, and problem-solving therapy.
• Provide education on medication and adherence, while monitoring any medication-related issues.
• Relay findings to Primary Care Providers (PCPs) and Psychiatric Consultants.
• Share treatment recommendations and updates among PCPs, patients, and psychiatrists.
• Align daily schedules with documentation in Mindoula's electronic medical record (MMR).
• Record all billable activities in MMR within 24 hours.
• Present follow-up cases to the psychiatrist during case presentations.
• Counsel patients regarding illness, abuse, medication, and community and social resources.
• Assess risks, create safety plans, and conduct crisis interventions.
• Monitor, evaluate, and document patient progress through validated monthly assessments.
• Prepare and maintain necessary records, reports, paperwork, and diagnostic documents while ensuring confidentiality.
• Communicate effectively with patients, families/caregivers, staff, and the treatment team.
• Produce high-quality documentation.
• Relate effectively to patients, families, and involved agencies.
• A Master's Degree in psychology, social work, or another behavioral health discipline from an accredited institution is required.
• Proficiency in technology to effectively work remotely, particularly with Google Suite (calendar, docs, sheets, slides, etc.).
• Strong experience or capabilities in documentation and data entry.
• Previous work experience with Electronic Medical Records is essential.
