Behavioral Health Care Manager

📋 Description

• Conduct an initial biopsychosocial intake for clinical members, which includes a safety assessment and the administration of behavioral health evaluations (PHQ-9, GAD-7, etc.).

• Participate in follow-up appointments and asynchronous communications, providing brief interventions such as behavioral activation, mindfulness, and psychoeducation.

• Serve as the primary care coordinator, acting as a conduit between the referring specialist and the practice, ensuring that member inquiries or administrative questions are addressed or directed to the appropriate team within Cerula Care.

• Facilitate weekly interdisciplinary team meetings with the Consulting Psychiatrist and Health Coach, ensuring that all high-risk members are reviewed and that new psychiatric recommendations are recorded in the patient charts.

• Maintain ongoing communication with the Cerula Care behavioral health team, including Coaches and Psychiatrists.

• Record member progress in a HIPAA-compliant electronic medical record system and client registry provided by Cerula Care.

• Identify patients who are not making progress and may require more intensive mental health services, reporting any concerns to the medical provider and Consulting Psychiatrist.

• Assist in referring patients for clinically necessary services outside of Cerula Care under the guidance of the BHCM Lead (e.g., SMI).


⛳️ Requirements

• An active, unrestricted independent clinical license in at least one U.S. state (e.g., LCSW, LICSW, LPC, LCPC, LPCC, LMHC, or LMFT). Employees must maintain licensure in good standing throughout their employment.

• MUST be bilingual in both English and Spanish.

• A bachelor's degree in nursing, social work, or other health and human services disciplines from an accredited institution is required; a master's degree is strongly preferred.

• A minimum of two years of clinical care management experience, with a portion of that experience directly involving work with individuals facing cancer or advanced illnesses (e.g., cancer coaching, palliative care, etc.).
