
Behavioral Health Care Manager

This is a fully remote position, open to applicants in North Carolina.
• Collaborate closely with the patient's primary care provider to ensure mental health care is well-coordinated and establish a mutually agreed-upon care plan.
• Conduct screenings and assessments for prevalent mental health disorders.
• Assist in managing psychotropic medications prescribed by PCPs, with an emphasis on treatment adherence, monitoring side effects and complications, and evaluating treatment effectiveness.
• Monitor patient follow-up and clinical outcomes utilizing a registry, document encounters within the registry, and leverage the system to identify and reconnect with patients.
• Develop specific mutual self-management goals, objectives, and action-oriented behavioral interventions with proactive patients.
• Adjust care plans for patients who are not showing improvement, which may include medication changes, psychosocial treatment modifications, or appropriate referrals for additional services.
• Engage in regularly scheduled consultations regarding caseloads with psychiatric consultants.
• Experience in providing care to individuals facing mental health disorders.
• Background in a team-oriented healthcare environment.
• Minimum of three years of clinical experience.
• Expertise in evidence-based treatment methodologies (Problem Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, Behavioral Activation).
• Familiarity with measurement-based care tools (PHQ9, GAD7, C-SSRS, etc.).
• Master's degree in a mental health-related discipline.
