This is a fully remote position, open to applicants in Texas.
• Develop, implement, and assess population health initiatives focused on medication management, adherence, and maximizing therapeutic outcomes.
• Leverage health data analytics to recognize trends, identify care gaps, and pinpoint opportunities for intervention.
• Collaborate with healthcare professionals to ensure proper medication selection, dosing, and monitoring.
• Design and execute programs to educate patients about their medications.
• Work closely with healthcare teams to synchronize care plans and interventions.
• Engage in quality improvement projects aimed at boosting medication-related outcomes.
• Provide patient care in an appointment-based model for both dispensing and non-dispensing patients.
• Remain informed about pharmaceutical regulations to ensure compliance.
• Oversee the hiring, training, and management of a team of Pharmacy Care Coordinators.
• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Valid pharmacist licensure in the state or region of practice.
• Advanced training or certification in population health management or a related field (preferred).
• Strong analytical skills and proficiency in data interpretation.
• Experience in pharmacy practice, ideally in a population health or managed care environment.
• Exceptional communication, collaboration, and leadership skills.
• Familiarity with healthcare regulations, policies, and quality improvement methodologies.
