This is a fully remote position, open to applicants in Alabama, +32 more states.
• Answer inbound calls and inquiries regarding the care and wellness program.
• Investigate previous claim activities, inquiries, and spousal claim matters.
• Initiate outbound calls to policyholders and their family members to inform them and promote program participation.
• Utilize standardized tools to thoroughly evaluate policyholders and/or their family members.
• Examine recommendations and persuade policyholders and family members to take necessary actions.
• Coordinate subsequent program steps with team colleagues according to policyholder requirements.
• Analyze claim intake calls and past claim details to comprehend current situations.
• Employ clinical judgment to assess the suitability of interventions and the need for additional care.
• Evaluate and address biological, psychological, environmental, and social needs.
• Apply active listening and motivational interviewing techniques during telephonic assessments.
• Integrate assessment findings and suggest suitable program interventions.
• Foster empathetic and professional relationships with policyholders and their care teams.
• Offer education on resources for aging, medication management, daily living aids, and prevalent medical conditions.
• Research resources, advocate for insured individuals, and provide initial caregiver coaching.
• Address routine inquiries from policyholders and family members using internal resources and escalation procedures.
• Organize and prioritize a workload and multiple responsibilities effectively.
• Operate independently and collaborate within an interdisciplinary team environment.
• Offer feedback for process improvements and mentor peers as necessary.
• Enthusiasm for working with older adults and their families.
• 2–5 years of professional experience with older adults and their families.
• A college degree or certification/license in a care-management-related field, or equivalent professional experience.
• Experience in recruiting, selling, or enrolling seniors in programs.
• Proven experience in conducting assessments for older adults and caregivers.
• Exceptional written and verbal communication and customer service abilities.
• Capability to meet weekly productivity standards while ensuring high quality.
• Ability to inspire individuals with post-operative musculoskeletal, neurological, and general acute issues.
• Experience in making informed business decisions adhering to risk management and quality protocols.
• Proficiency in working efficiently both independently and within a team setting.
• Strong skills in building relationships with both internal and external stakeholders.
• Eagerness to learn about Long Term Care Insurance contracts and benefits.
• Comprehension of medical conditions that are common in aging and their impacts on physical/cognitive function, prognosis, and rehabilitation.
• Familiarity with Word, Excel, and PowerPoint.
• Willingness to learn claims and workflow systems.
