Advanced Practice Clinician – On-Call, Provider of the Day

This is a fully remote position, open to applicants in New Jersey, +2 more states.

📋 Description

• Act as the Provider of the Day and serve as the after-hours On-Call provider for a high-risk geriatric demographic.

• Conduct both scheduled and urgent telemedicine consultations, as well as same-day virtual assessments.

• Address immediate patient concerns and deliver remote symptom management.

• Analyze laboratory and diagnostic results while coordinating follow-up care.

• Assist providers and nursing staff with acute-care inquiries, treatment suggestions, and escalated triage situations.

• Oversee medication refill requests and other urgent clinical requirements.

• Aid in transitions of care and follow-up after hospital discharges.

• Enhance patient communication and care coordination.

• Provide overnight coverage on weekdays and 24-hour availability during weekends and holidays throughout the assigned week.

• Assess the need for escalated care, including emergency evaluations when appropriate.

• Support patients, families, facilities, and staff during urgent circumstances.

• Record all after-hours interactions in the Electronic Health Record (EHR).

• Collaborate with providers, nurses, and operational teams to ensure high-quality care.

⛳️ Requirements

• Completion of a recognized Nurse Practitioner or Physician Assistant program.

• Active and unrestricted NP or PA license required.

• Must possess licensure in Pennsylvania, New York, New Jersey, and Delaware.

• Willingness to obtain licensure in additional states as the organization grows.

• Preferred experience in geriatrics, family medicine, internal medicine, or emergency medicine.

• Previous telemedicine experience is preferred.

• Strong clinical judgment and the ability to work autonomously.

• Excellent skills in communication, organization, and problem-solving.

• Comfortable managing complex geriatric patients remotely and in collaboration with others.

• Ability to excel in a fast-paced, team-oriented environment.
