Virtual Family Nurse Practitioner, Michigan Licensed

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Review the daily caseload and prepare necessary technology and tools for patient services.

• Utilize modern technology to manage the daily schedule effectively.

• Collaborate with clinical in-home team members during visits to patients' residences.

• Foster welcoming relationships with a diverse range of patients.

• Conduct virtual wellness visits for adults and/or pediatric patients.

• Execute health risk assessments, medication evaluations, and cognitive screenings.

• Provide patients with educational resources related to their health and well-being.

• Assess vital signs using Sprinters and organize blood draws.

• Address specific patient needs through tailored tasks.

• Coordinate comprehensive care, incorporating preventive measures, medication management, specialist referrals, community resources, and documentation.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Possess an active Family Nurse Practitioner License in Michigan.

• Exhibit high levels of professionalism, integrity, accountability, confidentiality, care, and compassion.

• Demonstrate a willingness to creatively solve problems in a dynamic environment.

• Be technologically proficient and comfortable using laptops, mobile devices, charting tools, and HIPAA-secure messaging applications.

• Possess strong written and verbal communication skills.

• Ability to work independently or collaboratively within a team setting.

• Must successfully pass a national background check and valid clinical license verification.

• Previous experience conducting wellness visits is required.
