Virtual Clinician, Home Evaluations, Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct virtual “Video Visits” with members of health plans.

• Interact with health plan members primarily through online platforms, assisting in addressing gaps in care.

• Ensure availability for 40 hours each week.

• Achieve a high visit completion rate.

• Adhere to operational, clinical, and member experience key performance indicators for compliance.

⛳️ Requirements

• Full-time, board-certified nurse practitioners or physician assistants are required.

• Prior experience with Signify Health In-Home Health Evaluations, Video Visits, and telehealth visit implementation.

• Must maintain training, affiliation, and adherence to Video Visit standards.

• Be certified and credentialed for all health plan clients in primary coverage states.

• Active state licenses in coverage areas must be maintained (FNP, AGNP certification preferred for NPs).

• Capability to obtain and sustain multi-state licenses as necessary.
