Virtual Advanced Practice Clinician, Chronic Care

This is a fully remote position, open to applicants in Colorado, +2 more states.

📋 Description

• Facilitate virtual onboarding, annual assessments, transition-of-care, and follow-up consultations for designated patient panels.

• Conduct comprehensive biopsychosocial evaluations and oversee the management of complex chronic illnesses such as hypertension, diabetes, COPD, and heart disease.

• Order and monitor laboratory tests and imaging studies.

• Provide health coaching, lifestyle guidance, and education tailored to specific diseases.

• Execute documented clinical transitions to longitudinal care teams.

• Oversee the centralized Float Pool inbox, which includes normal results, crucial medication refills, abnormal-result notifications, and issue routing.

• Offer urgent clinical triage and manage results during provider absences.

• Assist in preventative-care quality initiatives and close gaps in HEDIS/STARs metrics.

• Mentor Health Advocates during virtual appointments.

• Collaborate with behavioral health, nursing staff, pharmacy, and external healthcare providers.

• Participate in weekly Multidisciplinary Care Conferences as required.

• Ensure precise and compliant electronic medical record documentation, diagnostic coding, and adherence to evidence-based care plans.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• A valid and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license.

• Active NP/PA licensure in at least one primary market: Michigan, Massachusetts, or Colorado.

• Openness to obtaining additional state licenses as needed.

• Current DEA registration along with a state-controlled substance license.

• Up-to-date BLS certification.

• A minimum of 3 years of experience in adult primary care, family medicine, urgent care, or emergency medicine.

• Proven experience in managing patients with complex chronic conditions.

• Proficiency with telehealth platforms.

• Capability to utilize emerging tools, software, and AI-driven clinical workflows.

• Preferred: experience in a high-volume virtual/telehealth care environment.

• Preferred: familiarity with value-based care, HEDIS/STARs, and clinical documentation improvement (CDI).

• Preferred: practical experience with AI tools in a clinical context.
