This is a fully remote position, open to applicants in Florida.
• Work in partnership with clinical practices to execute clinical initiatives aimed at enhancing the quality and value of cardiovascular care while continuously improving the operational performance and efficiency of these initiatives.
• Oversee contractual and quality metrics, establish metric targets, and formulate mitigation strategies to enhance contractual performance when these targets are at risk.
• Provide training to clinicians and practice staff on how to implement care model operational processes.
• Continuously evaluate and enhance the operational processes of care model programs in collaboration with Novocardia leadership and practice staff.
• Lead regular clinical quality meetings with practice leadership to assess clinical and operational outcomes, collaborate on unstable and high-risk patient outliers, and develop and implement quality improvement plans.
• Maintain close collaboration with the care management team director, ensuring alignment in goals and outcomes.
• Collaborate with data and analytics teams to design and oversee clinical outcomes and dashboards.
• Work with Account Management to monitor and achieve payor-facing contractual metrics.
• Manage priorities and strategies for clinical programs, quality measures, and other patient-centered value initiatives, adhering to industry standards, benchmarks (e.g., HEDIS, Stars), and contract requirements to ensure success across the company’s diverse portfolio of contract types and populations (Medicare, Medicaid, Commercial).
• Collaborate with the senior leadership team to formulate, communicate, and implement Novocardia’s strategic vision.
• Coordinate among product, operations, and clinical teams to ensure smooth workflows and clear deliverables.
• Identify market barriers and collaborate with the clinical services director to develop new solutions, refine clinical workflows, and enhance quality.
• Bachelor’s degree in nursing, healthcare administration, or another clinical field.
• A minimum of 3 years of experience in value-based care operations.
• Knowledge of value-based care clinical models, including quality metrics, regulatory environments, and best practices is essential.
• Proven experience working with practices and providers to promote success in value-based care arrangements.
• Expertise in delivering and managing the quality and process improvement cycle.
• Experience in interpreting and modifying data dashboards to ensure optimal patient outcomes.
• Significant experience in developing, monitoring, and driving performance against key performance indicators in clinical delivery environments.
• Experience in clinical product organizations with a demonstrated ability to develop and implement new clinical products.
• Exceptional organizational skills along with strong written and verbal communication abilities.
• Willingness to travel extensively to clinical practices.
• Experience in change management is highly preferred.
