This is a fully remote position, open to applicants in Florida.
• Manage incoming phone calls and ensure that the needs of callers are addressed effectively through exceptional customer service skills.
• Offer telephone guidance and education tailored to patient requirements.
• Respond to phone inquiries regarding the services offered by Chapters Health System promptly, courteously, and efficiently.
• Advocate for and elucidate the advantages of Chapters Health System’s services.
• Collect essential information and documentation to uphold comprehensive, up-to-date, and precise patient records.
• Input and monitor patient referral data within the electronic medical record system.
• Provide reassurance to patients and their families over the phone and assist in resolving any inquiries they may have.
• Identify when an in-person visit is necessary and facilitate the coordination of that visit.
• Support patients and families in achieving their desired levels of comfort.
• Utilize appropriate support and expert resources to address complex situations.
• Record interactions with patients and families in the EMR and communicate with the Interdisciplinary Group.
• Promptly report technical issues to leadership and strive for resolution within 30 minutes.
• If technical issues remain unresolved, report to an IPU or Resource Center as instructed.
• During emergencies, report to a company-designated work location as required.
• Carry out additional duties as assigned.
• A valid Florida or multi-state nursing license as an RN.
• Capability to apply for or secure a multi-state RN license within 30 days of hire if not already held.
• Ability to apply for or obtain an RN license to practice in Washington D.C. within 30 days of hire.
• At least one year of nursing experience.
• Preferred experience in hospice or hospital settings.
• Prior experience with an EMR/EHR system is desirable.
• Proficient in verbal and written communication in English.
• Bilingual in Spanish is a plus.
• Strong critical decision-making and problem-solving abilities.
• Excellent clinical and assessment capabilities.
• Competence in using equipment that has visual and auditory mechanisms.
• Exceptional time-management and prioritization skills.
• Ability to professionally and calmly manage stressful situations.
• Strong skills in patient service, information-seeking, probing, listening, understanding, and responding.
• Capacity to thrive in a fast-paced environment while maximizing caller reassurance and patient visits.
• Ability to collaborate effectively within a team environment.
• A sense of urgency in everyday work activities.
• Proficiency in computer operations and MS Office software applications.
• Skills in online research, data entry, and system navigation.
• Availability to work scheduled shifts during extended operating hours.
• Ability to perform sedentary tasks with frequent interruptions.
• Capability to manage emotional stress when caring for terminally ill patients and their families.
• Ability to execute essential physical job functions, including lifting, bending, pushing/pulling, and kneeling, with or without reasonable accommodations.
• Successful completion of position competency requirements.
• Consent to post-offer and ongoing drug/alcohol testing.
• Eligibility verification through the Florida Care Provider Background Screening Clearinghouse.
