This is a fully remote position, open to applicants in New Jersey.
• The Advanced Practice Provider will be tasked with delivering patient care, accurately maintaining patient records, and managing the scheduling and administration of follow-up appointments across various markets.
• Key responsibilities include evaluating patient health, developing strategies for health improvement or management, and promoting healthy habits.
• Providing education to patients regarding preventative care and prescribed treatments.
• Performing in-home assessments, which encompass comprehensive annual wellness exams, targeted screenings, and other visits in both the patient’s home and virtual settings.
• Advising and educating patients and their families about available benefits and programs designed to support healthier living.
• Overseeing care coordination with primary care providers, specialists, and relevant ancillary services.
• Ensuring all documentation and paperwork is completed promptly.
• Upholding quality care standards as established by the practice.
• Delivering evidence-based, timely care in a way that minimizes avoidable hospitalizations, enhances quality of life, and prioritizes patient health and satisfaction.
• Must hold an active and unrestricted Registered Nurse and Nurse Practitioner Board certification relevant to licensure (NP: ANCC/AANP).
• Current and unrestricted DEA certificate is required.
• Willingness to travel up to 100% of the time.
• Ability to operate independently and practice without direct supervision.
• Access to reliable transportation, a valid driver's license, and appropriate car insurance.
• Proficiency with medical instruments and equipment necessary for the role.
• Familiarity with computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
• Effective communication skills, both verbal and written, with various stakeholders including retail and medical partners, patients, family members, physicians, and community representatives.
• Strong understanding of all federal and state regulations, including HIPAA and OSHA.
• Experience with Managed Care/IPA/Health Plans.
• Background in conducting wellness visits, focused assessments, or similar comprehensive evaluations either virtually or in-home.
