Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Perform thorough health assessments through telehealth platforms

• Record risk adjustment (HCC coding) during patient consultations

• Address HEDIS care gaps during appointments

• Evaluate medical history, medications, and preventive requirements

• Record visits using ICD-10 and CPT II coding systems

• Provide clear care plans and follow-up instructions

• Ensure accurate documentation of visits for quality initiatives

⛳️ Requirements

• Current Nurse Practitioner license in Alabama

• Over 1 year of experience in Family Medicine or Internal Medicine

• Experience working with Medicare and Medicaid patient populations

• Knowledgeable in HEDIS and risk adjustment processes

• Required enrollment as a Medicare and Medicaid provider

• Proficient in delivering care via telehealth platforms
