This is a fully remote position, open to applicants in Arizona.
• Respond to referrals and connect with clients within 24 hours via home visits or phone calls.
• Complete documentation related to contacts and case information.
• Engage in ongoing outreach through phone calls, home visits, and collaboration with the treatment team.
• Gather and review collateral information such as social histories, psychological evaluations, judicial records, treatment details, and DPS reports.
• Provide continuous support and case management for individuals facing substance use challenges.
• Create personalized case management plans and manage substance abuse assessments.
• Develop and execute service plans, community referrals, reassessments, and follow-ups.
• Coordinate client services, advocacy efforts, and community resource information.
• Record services provided, write progress notes and summaries, and finalize monthly progress reports and billing.
• Facilitate Intensive Outpatient and Outpatient Groups utilizing evidence-based curricula.
• Offer advice and support to individuals and groups dealing with substance abuse issues.
• Complete ASAM, ASI, CIWA, CINA, and ACES assessment tools within specified deadlines.
• Organize, facilitate, and participate in Adult Recovery Team meetings.
• Advocate for quality improvement and undertake additional assigned responsibilities.
• A minimum of a high school diploma or equivalent is required.
• At least three years of relevant experience working with multi-problem families in substance abuse, child welfare, or behavioral health settings.
• An Associate’s degree in human services or a behavioral health-related field is preferred.
• Understanding of human behavior, social work theories and techniques, and community resources.
• Proficient in counseling, interviewing, assessment, and building therapeutic relationships.
• Ability to adapt to the unique needs of each client.
• Awareness and sensitivity to cultural differences.
• Proficiency in Microsoft Office applications, including Word and Outlook.
• Capability to stoop, bend, squat, sit on the floor, and navigate uneven surfaces, stairs, and steps.
• Ability to visually and auditorily assess and interact with clients to ensure their safety and wellbeing.
• Willingness to work outside the office, drive extensively, be available for occasional evenings and weekends, and respond to on-call emergencies.
• Capacity to lift up to 25 pounds.
• A valid Arizona driver’s license, reliable transportation, and proof of vehicle registration and insurance.
• Level One Fingerprint clearance or the ability to obtain it, along with the capacity to maintain this clearance.
• Must be at least 21 years old to drive on behalf of Catholic Charities.
