Social Worker

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Contribute to the design and implementation of Our Company's comprehensive, evidence-based virtual-first cardiac recovery initiative.

• Deliver virtual psychotherapy sessions (45-50 minutes each) to a group of patients.

• Provide therapeutic support for issues such as Depression, Nutrition, Smoking Cessation, etc., while promoting a holistic interpretive care approach.

• Assess patient symptoms and risk factors to establish diagnoses and craft personalized treatment plans.

• Ensure that treatment plans are aligned with standard practices and/or the latest research in the field.

• Adhere to the established plan of care to fulfill responsibilities within defined policies, procedures, and scope of practice.

• Reassess treatment plans as necessary to ensure patient safety and maximize benefits.

• Encourage patient engagement and successfully meet both patient and program objectives.

• Make autonomous decisions and adjustments to treatment in accordance with professional and regulatory guidelines.

• Guide patients using behavioral change methods (e.g., mindfulness, motivational interviewing).

• Communicate effectively with Our Company's integrated clinical team and partner health systems, addressing any unusual or urgent situations.

• Maintain complete, accurate, and timely records of patient sessions.

⛳️ Requirements

• Master's Degree or higher (required).

• Minimum of 3 years of mandatory Remote/Virtual experience or in Digital Healthcare.

• Illinois LCSW license from an accredited institution.

• Willingness to acquire licensure in additional states.

• Availability to work Monday to Friday from 8 AM to 5 PM.

• Evening availability via messaging platform is required.

• At least 3-5 years of experience practicing Cognitive Behavioral Therapy (CBT).

• Bachelor's degree in Psychology or a related field.

• Experience in Cardiac Health is preferred.

• Must be tech-savvy, passionate, and experienced in psychotherapy.

• Should possess an innovative mindset and the willingness to challenge traditional cardiac rehabilitation methods.
