• Develop and revise personalized service plans for seniors seeking home care, utilizing their personal and medical information.

• Keep clear and organized documentation for multiple active cases.

• Confirm that all plans adhere to regulatory and program standards by meticulously reviewing each plan before submission.

• Collaborate with the designated Service Coordinator to ensure a seamless process and accurate planning.
