Service Coordinator – Flexible Terms

This is a fully remote position, open to applicants in New Zealand.

📋 Description

• Facilitating service coordination for clients with various physical, intellectual, and sensory requirements.

• Organizing customized support packages that align with funding guidelines and individual aspirations.

• Establishing strong relationships with clients, whānau, service providers, and teams throughout the organization.

• Ensuring high-quality documentation is maintained using the Socrates IT system.

• Operating remotely from any location in Aotearoa, including Auckland, Wellington, Christchurch, Dunedin, or the surrounding regions.

⛳️ Requirements

• Demonstrated NASC experience in the health or disability sector.

• Strong familiarity with Socrates and funding allocation processes.

• Experience in coordinating both natural and formal supports for individuals with diverse needs.

• Exceptional communication skills with the capability to engage with varied cultural backgrounds.

• Keen attention to detail, strong organizational abilities, and confidence in working independently.
