Senior Patient Care Integrator

This is a fully remote position, open to applicants in United States.

📋 Description

• Integrate and manage all facets of patient care requests.

• Act as the primary contact for patients, coordinating with providers, specialists, and the care team.

• Oversee and ensure the accuracy of Electronic Medical Records (EMRs).

• Organize medical testing, procedures, and follow-up care.

• Create customized care summaries and medical briefs for patients.

• Tailor and distribute Personal Medical Kits according to patients’ medical needs.

• Engage in the patient-centered on-call rotation.

• Contribute to various patient care initiatives and responsibilities.

• Ensure adherence to state and federal healthcare regulations, including HIPAA.

• Assist the care team during unexpected or complex situations.

⛳️ Requirements

• An Associate’s degree in nursing, social work, or a related healthcare field is mandatory.

• RN credential is preferred.

• A minimum of five years of experience in a healthcare environment is required.

• Demonstrated experience in patient care coordination and leadership roles.

• Capability to manage and integrate complex care plans effectively.

• Strong organizational abilities and meticulous attention to detail.

• Exceptional written and verbal communication skills.

• Strong creative problem-solving abilities.

• Commitment to integrity and patient health-information confidentiality.

• Proficient in MS Office, Excel, and electronic health records systems.

• Ability to perform fine-motor tasks using computers and office or medical equipment.

• Strong visual acuity for reviewing patient records and clinical data.

• Capacity to work at a computer for extended durations.

• Ability to lift up to 25 pounds as necessary.

• Willingness to travel overnight for training and/or meetings.

• Must possess a valid driver’s license in good standing.
