This is a fully remote position, open to applicants in Missouri.
• Utilize evidence-based medicine and clinical expertise to assess the appropriateness of pharmacotherapy for high-risk patients suffering from chronic conditions such as diabetes, hypertension, heart failure, and COPD.
• Implement clinical protocols in accordance with organizational and state regulatory standards.
• Enhance drug coverage benefits to facilitate optimal care while minimizing costs (including preferred formulary products, cost-effective alternatives, and home delivery pharmacy services).
• Collaborate with physicians, nurses, and other healthcare professionals to improve patient care.
• Engage in high-risk rounds with a multidisciplinary team to review patient progress and modify treatment plans as necessary.
• Demonstrate exceptional patient counseling abilities and effectively convey complex medical information to both patients and healthcare providers.
• Educate patients about disease states, focusing on the proper use of medications and self-management techniques.
• Participate in quality improvement initiatives aimed at enhancing medication safety and efficacy.
• Conduct post-discharge medication reconciliation for patients transitioning from high-risk care.
• Maintain comprehensive documentation of all activities within the clinic's EMR and other required pharmacy or clinical management systems.
• Support CMS Stars and HEDIS quality measures to ensure successful clinic performance.
• Keep precise records of clinical interventions and patient care activities.
• Facilitate in-service education and training sessions for healthcare staff, while developing and distributing educational materials relevant to pharmacotherapy.
• Stay informed about advancements in pharmacotherapy and clinical guidelines.
• Offer consultation to healthcare providers regarding complex medication management issues.
• Address additional requests from the Primary Care Organization, providing support within the pharmacist's defined scope of practice to further organizational objectives.
• Implement targeted initiatives or priorities as directed by the National Director or Associate Directors of Pharmacy, adhering to best practices and regulatory standards.
• Active and unrestricted pharmacist license in Missouri.
• Willingness to obtain licensure in additional states for market support.
• Pharm.D. or equivalent advanced pharmacy degree.
• A minimum of 5 years' experience as a practicing pharmacist.
• Proven experience working in a team-oriented environment and contributing to shared goals.
• Completion of a clinical residency in ambulatory care or a related field (preferred).
• At least 2 years of experience in an ambulatory care or primary practice setting (preferred).
• Board certification in geriatric pharmacy or pharmacotherapy (preferred).
• Familiarity with electronic medical record (EMR) systems (preferred).
• Capability to identify and implement strategies for improved efficiencies and outcomes (preferred).
• Multilingual proficiency, preferably in English and Spanish (preferred).
