This is a fully remote position, open to applicants in Pennsylvania.
• The Health Coach conducts telephonic outreach to our Medicare demographic to enhance medication adherence and improve HEDIS diabetes and blood pressure control.
• Educates members and addresses barriers to adherence by leveraging the benefits available to them.
• This role may involve outreach to pharmacies and healthcare providers.
• Employs a collaborative approach that includes assessment, planning, implementation, and coordination to engage, educate, and influence members in their decisions aimed at achieving and sustaining optimal health.
• Through motivational interviewing and various tools, assesses members' health status concerning HEDIS health screening/control or Medication Adherence, utilizing healthcare expertise and knowledge of health risk behaviors.
• Utilizes motivational interviewing and influence skills to ensure member engagement and identify health needs through targeted questions and discussions to promote behavioral changes for optimal health.
• Collaborates with members to pinpoint opportunities for integration into available internal and external programs.
• Employs a holistic approach while consulting with the Manager or other colleagues to address barriers in assisting members in achieving their goals and objectives.
• Uses active listening and interviewing techniques to provide coaching, information, and support that empowers members to make sustainable, independent healthy healthcare choices.
• Interprets and applies guidelines/criteria to positively influence members' health outcomes.
• Supplies current healthcare information to aid members in understanding their health status.
• Encourages members to actively and knowledgeably engage with their healthcare providers in decision-making.
• Exercises judgment in implementing strategies aimed at mitigating risk factors and barriers while addressing complex health and social issues that affect healthcare access, services, and resolution of member challenges.
• Formulates a proactive action plan to tackle issues presented, enhancing both short- and long-term health outcomes as well as opportunities for improving a member’s overall wellness through integration.
• 1-3 years of experience with Microsoft Office.
• 3-5 years of practice experience in Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology Managed Care, and Medicare.
• Preferred healthcare licensure and/or certifications such as RN, LPN, MSW, LCSW, CDE, etc.
• 5 years of practice experience in Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology Managed Care, and Medicare.
• Previous experience in telephonic coaching, education, or case management.
• Proficient computer skills, including MS Office applications.
• Ability to work independently.
• Strong time management skills.
• Excellent analytical and problem-solving abilities.
• Strong communication, organizational, and interpersonal skills; bilingual skills are a plus.
• Proven ability to communicate effectively, solve problems, and work efficiently with individuals, exercising good judgment in challenging scenarios.
• Experience facilitating positive behavioral changes in clients.
• Background in motivational interviewing and active listening techniques.
