This is a fully remote position, open to applicants in Kentucky, +4 more states.
• Collaborate with telehealth providers to guarantee that suitable patients receive telemedicine services.
• Communicate effectively with onsite nursing teams to ensure seamless coordination of all clinical needs.
• Manage the provider queue in the Never Alone system.
• Assist with any technical issues encountered by the telehealth provider team.
• Supervise the EMR to triage regular clinical lab requests, routine imaging results, notifications, and other after-hours inquiries.
• Ensure timely documentation of interactions with facility nurses in the EMR.
• Input information directly into the CRM system.
• Connect patients requiring acute care from an in-house provider the following day with Central Scheduling.
• Respond to and triage calls related to Never Alone use cases beyond the SNF framework.
• Offer health education to patients.
• Deliver an exceptional customer experience to promote high levels of customer satisfaction and retention.
• Adhere to ethical practices in line with the Scope and Standards of Practice of the profession and Board Certification.
• Comply with all relevant state and federal regulations, guidelines, and laws.
• Work collaboratively with the telehealth provider group.
• Participate in monthly staff meetings.
• During downtime, there may be opportunities for additional project-related tasks, including protocol development, initial quality review of past encounters, or other projects as the role develops.
• Valid RN or LPN license is required.
• Willingness to obtain licensure in the 7 states served (Indiana, Ohio, Maryland, Virginia, West Virginia, and Kentucky) is necessary.
• Strong communication and critical thinking abilities are essential.
• Good aptitude for technology is required.
• Training or clinical experience in geriatrics, primary care/internal medicine, and/or emergency department or urgent care is preferred.
• A minimum of 3 years of clinical experience is required.
• Openness to professional development through training, obtaining necessary certifications, and attending team meetings is expected.
• Experience in a collaborative healthcare environment aimed at driving positive outcomes and achieving goals is beneficial.
• Training or experience in an outpatient primary care setting is advantageous.

• Option for an 8-hour shift
• Flexibility to select 1 shift per month and maintain a consistent schedule
