• Conduct virtual assessments of patients, including gathering medical history and performing health evaluations.
• Assist in the creation and organization of patient care plans.
• Execute medication reconciliation to ensure accuracy in medication management.
• Coordinate follow-ups, check-ins with patients, and address ongoing care requirements.
• Aid patients in navigating Medicare and assist with provider research.
• Monitor patients' progress and maintain timely documentation of their care.
• Manage a caseload of patients and oversee monthly care coordination activities.
• Collaborate with patients, caregivers, healthcare providers, home health teams, and other involved parties.
• Ensure continuity of care through effective communication and organizational skills.
• Maintain precise records and documentation of patient care.
• Provide scheduling and administrative assistance to the CEO.
• Organize transportation and logistics for patients as necessary.
• Assist with operational and executive-level responsibilities.
• Independently handle tasks and proactively initiate responsibilities.
• Utilize the company’s proprietary AI-assisted healthcare software for conducting patient interviews, structuring care plans, and managing documentation workflows.
• Ensure that documentation is accurate, timely, and well-organized.
• Uphold confidentiality and professionalism in all interactions with patients.
• A minimum of 2 years of nursing experience is required.
• Registered Nurse (RN) designation is preferred.
• Strong skills in clinical assessment and patient care planning are essential.
• Experience in care coordination, patient follow-ups, and remote support or virtual healthcare is necessary.
• A solid understanding of healthcare workflows and patient care systems is required.
• Excellent English communication skills, both written and verbal, are crucial.
• Highly organized with the ability to multitask effectively.
• Preferred Experience:
• Experience in care management or patient advocacy.
• Familiarity with Medicare workflows and patient support.
• Background in working with vulnerable or high-needs patient populations.
• Experience with healthcare technology platforms and remote documentation systems.
• Experience collaborating directly with providers, caregivers, and healthcare stakeholders.
