This is a fully remote position, open to applicants in Illinois.
• Foster meaningful connections with patients and their families, gaining insight into their objectives, challenges, and motivators.
• Develop tailored care plans that extend beyond mere diagnoses, focusing on social determinants and emotional health.
• Oversee care coordination among various providers and settings to ensure safety, quality, and continuity of care.
• Educate and empower members to make knowledgeable decisions and confidently navigate the healthcare landscape.
• Collaborate with a multidisciplinary team, exchanging insights and advocating for holistic patient health.
• Leverage data as a resource for insights—rather than a determinant of care—to uncover opportunities and evaluate impact while keeping the patient at the forefront.
• Ensure adherence to regulatory standards while prioritizing compassion and connectivity.
• Current RN license.
• Delegated Services – Illinois licensure is mandatory.
• Employee Health Plans – Compact licensure and/or single-state license as applicable.
• Value-Based Care – Compact licensure and/or single-state license as applicable.
• Strong experience in care management, particularly with complex and chronic conditions.
• Proficient in motivational interviewing, patient education, and advocacy.
• Capacity to operate independently in a remote setting while maintaining team connectivity.
• Comfort with technology and various EMR systems.
• Minimum of five (5) years of clinical experience across a range of healthcare environments.
• Behavioral Health: Requires advanced knowledge of psychiatric conditions, crisis intervention, and therapeutic communication; or
• Pediatrics: Clinical expertise in caring for medically fragile children, technology-dependent care, and family-centered methods; or
• Maternal-Fetal Health (Obstetrics): Clinical proficiency in managing labor and delivery, preferably in high-risk populations, fetal monitoring, and recognizing emergency obstetric interventions.
