This is a fully remote position, open to applicants in United States.
• Facilitate and coordinate high-quality, cost-effective care for patients with ESRD and CKD.
• Reduce fragmentation within the healthcare delivery system.
• Oversee care coordination among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager.
• Integrate and manage care for patients in the late stages of CKD and ESRD.
• Evaluate patient needs and create, as well as oversee, personalized care plans.
• Coordinate services across various interdisciplinary teams.
• Assist patients during complex transitions in healthcare.
• Manage clinical outcomes, financial results, and patient satisfaction metrics.
• Act as a change agent and implement CQI strategies.
• Offer performance coaching in healthcare settings.
• Adjust personal practice to accommodate new electronic processes and heightened productivity expectations linked to Capella implementation.
• An ADN degree from an accredited nursing school is required; a three-year diploma from an accredited program may be accepted as a substitute.
• A BSN is preferred.
• A current Registered Nurse (RN) license in the applicable state is mandatory.
• Continuing education credits must be maintained as mandated by the state of practice.
• A minimum of five years of experience in clinical nursing is highly preferred.
• At least three years of experience in renal nursing is preferred.
• Experience in case management is required, or alternatively, three or more years in assessing patient needs, creating and managing individualized care plans, coordinating interdisciplinary services, and supporting patients through complex healthcare transitions.
• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
• Proficiency in understanding and utilizing data to manage clinical, financial, and patient satisfaction outcomes is essential.
• Experience in a change agent capacity is required.
• Familiarity with CQI techniques is necessary.
• Previous experience in healthcare performance coaching is required.
• A current CPR certification is mandatory.
• Must be able to adapt to new electronic processes and increased productivity expectations associated with Capella implementation.
• A valid driver’s license in the state of employment, a positive driving record, and the ability to meet insurance coverage criteria are required.
• Basic computer skills and proficiency in MS Word and Outlook are essential.
• Functional proficiency with DaVita clinical software programs, including Capella, is required within 90 days of employment.
