• Conduct home visits with participants at least every six (6) months, adhering to established schedules.
• Exhibit a thorough understanding of the Personal Options services program, relevant West Virginia waiver programs, and Palco policies.
• Provide education and mentorship to participants and/or their designated representatives regarding their rights, responsibilities, and risks associated with the program.
• Assess and evaluate the capability of participants or their designated representatives to engage in the program.
• Support members and representatives in creating and updating their initial service/spending plan as necessary.
• Monitor the utilization of service/spending plans.
• Address and respond to all complaints and grievances from members/representatives concerning FMS and Resource Consulting services.
• Act as a mandatory reporter, promptly reporting and addressing all required incidents, including allegations of suspected abuse, neglect, and exploitation.
• Employ a person-centered approach during all interactions with participants.
• Meticulously document visits with participants using a company-issued laptop, utilizing professional and detailed language.
• Record events and generate reports electronically.
• Complete necessary administrative tasks daily, including scheduling and confirming visits and responding to company emails and calls.
• Adhere to operational procedures as outlined by Palco.
• Engage in shadowing opportunities with new resource consultants upon request and participate in required periodic training sessions by Palco.
• Timely distribute new information to participants within your caseload.
• Handle company equipment and data with care in compliance with company policies, including HIPAA privacy practices.
• At least three years of experience in public-facing roles is mandatory.
• Preferred experience includes roles such as patient navigator, community connector, community health worker, lay health advisor, health educator, or case manager.
• A High School Diploma or equivalent is required.
• Travel is necessary for this position.
• Strong verbal and written communication skills are essential.
• Ability to document visits electronically is required.
• Proficient in Microsoft Office, Microsoft Outlook, and Internet usage.
• Strong problem-solving abilities are a must.
• Willingness to work with individuals of all ages and disabilities is essential.
• A valid driver’s license and reliable transportation are required.
• Must maintain an active prepaid or contract cell phone plan for ongoing communication with participants, management, and/or company leadership.
• Ability to pass a criminal background check and maltreatment check is required.
