This is a fully remote position, open to applicants in Canada.
• Identify the necessary rehabilitation assessments to evaluate the Participant’s potential for improvement, clarifying rehabilitation requirements and coordinating as needed.
• Ensure that services and plans are tailored to individual needs, goal-oriented, and aligned with program mandates aimed at enhancing the six functional domains of well-being while minimizing or removing barriers to re-establishment, ensuring safety, cost-effectiveness, and high quality.
• Create comprehensive Rehabilitation Plans for program Participants in collaboration with the Participant, their family or caregiver, Rehabilitation Service Professionals (RSPs), and the Veterans Affairs Canada (VAC) Case Manager.
• Serve as the service integrator by working equally with medical, psycho-social, and vocational RSPs who provide services to participants.
• Empower Participants to explore community-based interventions and resources to facilitate problem-solving and achieve their desired outcomes.
• Recognize barriers or risk factors that may lead to suboptimal outcomes and proactively design and implement effective strategies.
• A bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field, along with one of the following: a health professional designation or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).
• At least three years of experience providing medical, psycho-social, or vocational rehabilitation services (or a combination of these).
• Experience supporting individuals facing mental or physical health challenges that create barriers in their lives.
• Employment is contingent upon obtaining federal security clearance – Reliability Status.
• Willingness to travel across the region as required.
• Bilingualism (French/English) is considered an asset.
• Experience collaborating with employers, community resources, and training institutions.
• Current and former members of the Canadian Armed Forces (CAF) are encouraged to apply.
